VNA Health Care provides comprehensive primary care and community health services. Services include: Health Center: primary care for all ages to treat acute illnesses, provide wellness and preventive services, well-baby checkups and immunizations, school physicals, women's health services, and pregnancy care and deliveries. Doctor house calls also available. Pharmacy: full-service pharmacy for VNA clients located in Aurora office. Home Health care includes in-home assessment and care coordination, specialty nursing care, rehabilitation therapies (physical, occupational and speech), nutrition support, social work help and personal care services. Hospice care: in-home support for patients and families seeking comfort care in the final stages of life, and grief support for family members; Community Health and Wellness includes: Health screenings for men and women (blood sugar, cholesterol, blood pressure, body fat), breast and cervical cancer screenings for women, flu shots; Women, Infants and Children (WIC) program; Healthy Families in-home support for parents of infants and toddlers, and an early literacy program for children receiving services through the VNA health centers. Call for an appointment or for information; walk-ins always welcome. Bring proof of income.

Program Fees:

Sliding scale to no fee. Medicaid, Medicare, and insurance is accepted. Minimum fee applies
