The Cook County Clerk's Office in Rolling Meadows is responsible for maintaining vital records (birth certificates, death certificates, marriage and civil union licenses and certificates, assumed name filings, notary filings and verification), voter registration, and poll watcher credentials as well as providing early voting, in-person absentee voting for the northwest suburban section of Cook County. Ethics and campaign disclosures as well as information on county board proceedings is handled by the central office in Chicago. Vital records can be ordered on-line, by phone, by mail or in person. Marriage and civil union licenses, notary public renewals and voter registration can be applied for at the office. Spanish is spoken.

Vital Records: 8:30 am-4:30 pm Monday-Friday;4:30 pm-7:00 pm Wednesday for Marriage and Civil Union Licenses only.
Elections: 8:30 am-4:30 pm Monday-Friday;4:30 pm-7:00 pm Wednesday.
